How to keep a sleep diary without turning sleep into homework

A simple record of sleep timing and daytime context, with a copyable template and an example that keeps estimates separate from facts.

On this page

A useful sleep diary records roughly when you slept, how the next day felt, and relevant daytime context. A notebook is enough. You do not need to calculate a nightly score or know the exact minute you fell asleep.

The NHLBI sleep diary guidance includes sleep and wake times, naps, daytime sleepiness, caffeine or alcohol, and exercise. A record can help explain a problem to a clinician; keeping one is not a treatment for insomnia.

Start with one question

Write your reason at the top: “I want to explain why workdays and days off feel different.” That is more useful than trying to collect every possible number. If a clinician has given you a diary, use their version so you record what they need.

Use estimates and label them. “Around midnight” is a better record than an invented exact time. An empty space means unknown, not that nothing happened. You do not need to repeatedly check the clock during the night to complete this worksheet.

Copy this morning note

  • Date and whether it was a workday or a day off.
  • Time you got into bed; estimated time you fell asleep.
  • Remembered awakenings, if any; final wake-up and getting-up times.
  • How the following day felt, in your own words.
  • Naps, caffeine or alcohol, movement, and anything unusual you want to mention.
  • One question to discuss, rather than a conclusion about the cause.

The downloadable bilingual sleep note is an Own Rhythm organizational worksheet, not a clinical assessment or a replacement for your clinician’s form.

An example of a useful entry

Illustrative entry, not a client case: “Tuesday, workday. Bed about 11:30; sleep time uncertain. Woke before the alarm and got up at 7. Felt sleepy in a morning meeting. Afternoon coffee; late work call. Question: does this pattern also occur on days without late calls?”

That entry preserves uncertainty. It does not prove the coffee or call caused the sleepiness. The next useful step is to look for a repeatable pattern and bring the question to the appropriate professional, rather than changing several habits to make a graph look better.

Keep the record proportionate

If recording feels like another performance target, simplify it or discuss that concern with your clinician. Store notes somewhere private; there is no need to publish your health history or share identifying information in a public comment.

For a separate look at the evening transition, read our wind-down guide. The diary describes what happened; a routine is a different question. For unwanted light, the eye mask and curtains comparison addresses that specific choice.

When a diary is not enough

If sleep problems interfere with daily life, seek medical assessment. Do not wait for a perfect record. This worksheet cannot determine the cause of persistent sleepiness or authorize driving when you feel unsafe. Own Rhythm can help organize a non-medical conversation about routine; diagnosis belongs with a qualified healthcare professional.

Sources support the general health context; individual actions still require appropriate context and safety limits.
